Private Consulting with Dr. Paul

Hello, my name is Dr. Paul, I’m a Medical Doctor, board certified psychiatrist, and a former Associate Professor with experience teaching not only patients and the public, but other physicians. I have treated over 15,000 patients over the years, so I'm fully qualified to talk to you about your problems.

Please read the questionnaire below to see whether Dr. Paul will consider you for a consultation. After your order is complete, you will receive this questionnaire by email for you to fill out and email back to Dr. Paul.

MindOS Consultation Questionnaire:

1. Do you currently take psychiatric medication?

2. Have you had 3-6 of the following for more than two weeks?

• Sleeplessness
• Low energy
• Loss of much appetite or significant weight loss
• Panic (hyperventilation, with rapid heart rate and a sense of terror, avoidance of crowds and public places) attacks.
• Mental Confusion or memory loss
• Talking or thoughts so rapid they are not understandable
• Little need for sleep and still feeling very energetic without further need for sleep
• Bizarre or uncontrollable behavior
• Severe irritability, violence, or trouble with the law
• Hallucinations (seeing things or hearing things that aren’t there)
• Delusions (belief in totally irrational things), paranoia
• Worry or obsessions so severe they are debilitating to the point of not being able to work or socialize
• A severe medical diagnosis or closed head injury with unconsciousness, such as stroke, seizures, head injury, severe high blood pressure, or heart, diabetic, thyroid or other biological problem

3. If you answered yes to three or more of the above, or yes to the first question, or yes to a prior major mental health diagnosis, you may want to meet with a local professional first, before considering MindOS consultation.

4. If NO to most, if not all the above, you may be considered for consultation. Please write down your history of:

1. Medical diagnoses
2. past or current medication
3. past history of relationships to the other gender, of family problems, relationship problems, endings, and grief.

5. If I do not accept your application for consultation, we will refund your payment… Dr. Paul.

EMAIL CONSULT: With this consult you will send Dr. Paul. extensive information on yourself and your life.  Dr. Paul. then emails you back just as extensively an consult with solutions/homework. This is a one time long letter you send to Dr. Paul that should  be three pages max with your most pressing questions/problems to solve. If I do not accept your application for consultation, we will refund your payment… Dr. Paul.  Price $300

PHONE CONSULT: With this consult you will receive  an hour comprehensive session with Dr. Paul's impression in detail and solutions that can be executed by you over several months. You will the write back three brief reports to Dr. Paul by email, and to which Dr. Paul. will respond with brief corrections. If I do not accept your application for consultation, we will refund your payment… Dr. Paul. Price $500

1 ON 1 CONSULT: With this consult you would be meeting with Dr. Paul. IN PERSON in Chicago, IL USA. Transportation and all other expenses are at your expense. You will receive a questionnaire after you order - please fill it out and email it back to Dr. Paul. If I do not accept your application for consultation, we will refund your payment… Dr. Paul.  Price $5,000
